Plan your visit

Step 1: Registration

You may register online or via email, mail or fax:

• Online: Register online through our secure website and review our financial policy. 一旦您完成审查，您将被引导到患者登记表.

Step 2: Medical records submission

Before we can schedule your appointment, we need to receive your medical records, which may include doctor's notes, radiology images (in DICOM format) and reports, pathology reports, surgical reports and lab results. You may send us your medical records via email, fax or mail. Please find our contact information below under "Contact us."

Step 3: Medical records review

Step 4: Financial clearance

Step 5: Appointment scheduling
